Police said Roy Clifford Baxter, 55, of Mount Maunganui, was driving the van when it crossed the centre line on State Highway 29 on Thursday about 4pm.
People in a Subaru vehicle, also involved in the crash, were still in hospital.
One was in a critical condition.
A 9-year-old girl, who was a passenger in one of the vehicles, died shortly after the crash.
